You will need the following information to fill out and receive credit for your course. (if you fill out your information through your account it will auto populate)
First name
Last name
Email address
Address
City
State
Zip
Phone Number
What state your license is held in?
License type
Your license number (The number on your state/local license)
Your license expiration date
*only needed if you have selected a NREMT certified license type*
*NREMT certification number
*NREMT Expiration Date
After filling in your information answer the 6 questions at the bottom of the survey then select "Submit"
